All The Formula staff are New York State licensed.

Booking Appointments

Please call us at 914-305-2424 to book all appointments. A credit card is required at the time of booking to hold all appointments.

Consultation Fees

A credit card is required at the time of booking to hold all appointments. Our consultations have a fee of $50 which will be applied to your services the day of your appointment. We have a 24-hour cancellation policy. There will be a charge of $50 for any missed appointments.

Payment Information

We accept American Express, MasterCard, Visa, Cash and Nicole Frontera gift certificates. We do not accept personal checks.

Tipping

Tips may be paid in cash only. 15% is customary, however, the amount you leave is truly at your discretion.

Punctuality

We respectfully request that you arrive 15 minutes prior to your scheduled appointment time. Please allow time for traffic and please call when you know you will be late. When booking services at The Formula, your appointment is scheduled with a team member, whose time is allocated exclusively for your scheduled treatment. If you are late for a scheduled appointment, our company policy offers you two options:
– You may opt to shorten your treatment but will still be responsible for the full cost of the treatment
– You may prefer to reschedule your appointment at a more convenient time, however, a $50 cancellation charge will be incurred

Cancellation Policy

We understand that occasionally you may have to cancel an appointment with us. However, we do ask that you give us fair warning. You will be called and/or texted 48 hours in advance to confirm your appointment. As a courtesy to our clients and staff, it is company policy for all clients to give 24 hours’ notice of cancellation. Failure to do so will result in a $50 charge of all scheduled appointments. Rescheduling an appointment on the same day of the appointment is considered a cancellation and will also result in a $50 charge.

Refunds

We are sorry, Skin Care products and Treatments are not refundable.

Membership Cancellation

There is a 6-month minimum contract with Memberships 1 & 2 and a 12-month minimum contract with Memberships 3 & 4. Cancellations prior to 6-month contract will be charged the retail price differentiation of redeemed services. New York State Sales Tax will be added to all memberships. Gratuities are not included in membership prices.
